Student Registration

Welcome to Putnam County Middle School


We are excited that your child will be attending Putnam County Middle School. 

The staff at Putnam County Middle School is committed to making the enrollment process as easy and efficient as possible for families. 

Putnam County Charter School System's new student registration is designed to assist families with student registration needs associated with transferring and relocating to the Putnam County Charter School System.

New Student Registration is Online ONLY 

1. Begin the Online Registration Process 

  • Birth certificate/Guardianship Paperwork 
  • Social Security Card 
  • Immunization (DHR Form 3231
  • Ear, Eye & Dental Form (GA Form 3300
  • Two (2) proofs of Residency 
    • Mortgage or Lease/Rental Agreement or
    • Two (2) Utility Bill (gas, electric, water, sewer, or trash within 30 days from date of registration date) 
  • Valid Photo Identification/Passport of Registering Parent

  • Once completed, you will receive an email confirmation within 24 to 48 hours of the submitted application with the steps to schedule a registration appointment. The required documents may be uploaded at time of registration or brought in during your appointment.

2. Attend a registration/NWEA-MAP Testing appointment (parent and student, by appointment only).

  • Monday through Thursday between the hours of 09:00 am and 1:00 pm 
  • Student will MAP Test during the registration appointments (around 2 hours depending on student progress).

3. Records are Requested by Registrar 

  • Student records sent by previous school must remain sealed. Any unsealed student’s records are not valid.

  • Records upload available during online registration 
  • Non-educational records are the responsibility of the parent.

4. Complete Registration 

  • Students will receive a start date and schedule once ALL records are received by the previous school and Map testing is completed. 

 

Any Parent/Guardian Withdrawing Students must come to the school and present valid photo identification AND return all equipment issued by Putnam County Charter School System (all equipment must be returned BEFORE records are released to a requesting school). ONLY THE ENROLLING PARENT CAN WITHDRAW A STUDENT.
